Weekly Fatality/Catastrophe Report
This table contains the weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers. Employers must report these incidents to OSHA within eight hours. The summaries below include only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans. The fatalities listed here include only those that initially appear to be work-related, but excludes fatalities that do not appear to be work-related, such as an apparent heart attack of a sedentary worker. OSHA investigates all work-related fatalities and catastrophes. After OSHA's investigation is complete, these reports will be updated with inspection results and citation information.

Worker was unloading household goods at a residential site when the loading ramp to the truck allegedly slipped causing the victim to fall and strike the side of the ramp resulting in internal injuries. The worker was admitted to the hospital where he underwent surgery for a lacerated spleen, kidney damage and punctured lung the following day. Prior to discharge from the hospital the worker sustained a massive heart attack. | |

The worker was cleaning the frame of a utility trailer in preparation for painting. The utility trailer was clamped/attached to the forks of a locally manufactured hydraulic lift. The worker raised the lift to its maximum height and while standing under the trailer, continued to hold the hydraulic pump lever in the up position. The hydraulic pressure was too great, a chain link broke, the lift and the 800-pound trailer collapsed on top of the victim. | |

Workers were installing a motor and gear box on a chemical process vessel that had been rinsed with a flammable liquid. The vapor from the cylinder was ignited by the welding causing an explosion, knocking three workers off of a 9-foot platform. Four workers were hospitalized. |
