Investigation Summary
At 2:30 p.m. on September 3, 2020, an employee was returning to his truck and walked between the truck and a stack of timber swamp/crane mats. The counterweight of a nearby excavator struck the top mat and caused the mat to fall onto the employee and pin the employee between the truck and the mat. The employee received severe crushing injuries to the chest and was killed.
