Investigation Summary
At 12:30 p.m. on September 26, 2018, an employee was observing a coworker who was driving a tractor with a baler. The coworker shut down the tractor and the employee began adjusting the area around the stuffer arm. The employee finished with the adjustments and cleared the stuffer. The coworker restarted operations and the stuffer arm began to cycle. The coworker saw the employee be struck in his head. The employee was killed from blunt force trauma to the head.
