Employee is crushed and killed by machine cab

At 9:15 a.m. on May 14, 2018, Employee #1 was performing maintenance on a piece of equipment. After greasing a machine, Employee #1 accessed the steps of the machine's cab without notifying the operator. For an unknown reason, the operator maneuvered the cab clockwise, knocking Employee #1 from the steps. Employee #1 fell beneath the cab, where he was crushed, suffering fatal injuries to his chest, shoulders, and neck. Employee #1 was killed as a result.
