Investigation Summary
At 4:30 p.m. on March 25, 2018, Employee #1 and #2 were clearing the fuel burner of large chunks of fuel. Carbon monoxide gas levels accumulated to a dangerous level, resulting with both employees being overcame by the fumes. Employees #1 died and Employee #2 was hospitalized.
