Investigation Summary
At 5:45 a.m. on June 09, 2022, Employee #1 was securing trailer tie downs for removal of granite curbstone slabs. On the opposite side of the trailer, a coworker was unloading a pallet of granite curbstone slabs with a skid steer. During the offload operation the adjacent pallet of slabs near Employee #1 was shifted and fell off of the flatbed trailer onto Employee #1. The slabs crushed the abdominal and lower body, killing Employee #1.
