Investigation Summary
At 3:00 p.m. on May 23,2018, an employee was putting grass clippings in a trailer using a John Deere 553 loader. The metal bucket was raised to the maximum height, detached from the boom arms, and fell on the employee. The employee was struck on the head and upper back by the bucket. The employee died. The loader implement (metal box bucket) was fabricated for grass cuttings. The locking pin which secured the metal bucket to the boom was not locked into place.
