Investigation Summary
At 4:15 a.m. on May 8, 2018, Employee #1 was tightening a screw on a wig-wam machine. The machine was energized and, during work, Employee #1 came in contact with the moving arms of the machine's stacker pendulum. Employee #1's head and arm became caught in the machinery, killing him.
