Investigation Summary
At 2:45 p.m. on September 19, 2019, an employee was securing a rope to trees in an attempt to move them from a deadfall located in a river and signaled to his coworker to pull forward. The employee was struck by a tree limb that broke off from the tree and flew through the air. The employee sustained multiple fractures to include shoulder blade, ribs, and a punctured lung. The employee was killed.
