Investigation Summary
At 10:45 a.m. on July 24, 2018, an employee was trying to stabilize stack of four bales weighing 500-700 pounds each. The bales fell and struck the employee, knocking him off his forklift and into the concrete floor. The employee received fatal head injuries and fractures.
