Investigation Summary

On February 20, 2018, at approximately 12:30 am, an employee driving a truck had stopped to fix the plastic tarp that had come loose, to prevent it from blowing away during transport. The employee parked on the side of the road, trying to fix the tarp on the load in the back of his truck. His coworker who had stopped his truck behind the employee fixing the tarp, got out of his truck to assist the employee but failed to place the parking brake. His truck rolled forward trapping the employee fixing the tarp between the two trucks. The employee died from crushing injury. The two cargo trucks both weighed over 2,000 pounds each.
