Investigation Summary
At 1:00 p.m. on March 4, 2018, an employee was using a forklift to lift a dock plate. The dock plate was tied to a truck with a chain. The forklift operator was lifting and pushing the dock plate and the truck driver was pulling on the plate. As the dock plate lifted from the pins, the truck driver continued to pull on the plate to slide the dock plate off of the fork blades, resulting in the forklift being pilled over the open dock to the ground. The employee jumped from the forklift and was pinned between the forklift and a high beam that was located against the warehouse dock wall. The employee suffered injury to his leg and chest area, killing him.
