Employee is struck in the face by ball hitch and is killed

On June 21, 2019, an employee was using a forklift to move campers when it became stuck in the mud. The employee asked a contractor to pull the forklift out of the mud using a tractor. They tied a tow string around the ball hitch and the contractor began to tow the tractor when the ball hitch came off and struck the employee in the face, killing him.
