Investigation Summary
At 4:30 a.m. on December 13, 2018, an employee was working with several coworkers in tearing off an old roof from a building. The employee moved to grab the insulation that was lifted by one of his coworkers. He stepped onto a rotted board, and he fell approximately 40 feet when the board gave way. The employee was killed.
