Investigation Summary
On November 20, 2017, an employee was cleaning the cab of a semi-truck cab, and was vacuuming the driver's seat. The semi-truck had two integrated steps to access the cab. The steps to the semi-truck cab were 15" and 32.5". The employee was standing on either the first step (15") or the second step (32.5"), when he fell to the floor. No one witnessed the incident. An outside vendor was working in a truck bay next to the incident and heard a noise. The outside vendor looked back and observed the employee on the floor, unconscious. The employee was transported to the hospital and passed away on November 21, 2017.
