Investigation Summary
At 10:00 a.m. on November 11, 2020, an employee was cleaning and disposing of leaves. The employee was dumping leaves over a wall when he had a heart attack, which caused him to fall. The employee collapsed onto a perimeter fence, fell onto a sidewalk, and fractured his neck.
