Investigation Summary
At 8:00 a.m. on November 14, 2019, an employee was assisting his coworker with removing labels from glass sheets and was pushing six sheets of glass back onto dolly when it moved, shattering the glass sheets. The employee was struck by shards of glass and sustained internal injuries. The employee was hospitalized for 16 days and died on December 1, 2019.
