Investigation Summary
At 2:00 p.m. on April 19, 2018, an employee was installing enclosures for exterior hoist operations. The employee peeked over the 8th floor landing gate to verify if green paint was applied to the exterior corner piece. At the same time, a hoist was lowered by a coworker from the 9th floor, which struck the employee in the head. The employee was killed as a result.
