Investigation Summary
At approximately 2:30 p.m. on November 12, 2018, an employee, a laborer, was dismantling a portion of the C7 Pastuerizer line in beverage facility. The employee was using a Stihl TS 420 Cut Off Saw (serial number is unknown at this time) equipped with a 14 inch Diablo metal cut off abrasive wheel to separate a portion of the stainless steel frame that had the company's nameplate welded onto it. While making one of the cuts, the cut off saw kicked back, striking him in the left shoulder and neck. A coworker, a manager, was approaching the employee when the accident occurred and assisted the employee down the steps where he then staggered and fell. The employee made it approximately 25 feet from the point of the accident before collapsing. Another employee, the job superintendent, called 911 and was directed on how to assist the employee as well as where to meet the ambulance. When EMS arrived on scene, they pronounced employee as dead on arrival.
