Investigation Summary
At approximately 3:30 p.m. on September 19, 2017, Employee #1 was working on an elevated vehicle. The vehicle was suspended on the forks of a forklift. While removing the vehicles cross member/motor mount to get to the rack and pinion. Employee #1 was struck in the head by a metal object, either the cross member/motor mount or the vehicle itself, when it fell. Employee #1 suffered blunt force head trauma and died.
