Investigation Summary
At about 1:04 p.m. on August 10, 2017, Employee #1 was tasked by the employer of a pest control company to remove and keep bats out of the attic, known as bat exclusion, of a two story house. There were two roof structures to the home. The upper roof was approximately 42 feet long by 25 feet wide and had three dormers. The lower roof was approximately 40' long by 12' wide. The upper roof was a steep roof and the lower roof was a low slope roof. The edge of the lower roof to ground was approximately 26 feet. Both roof tops were constructed with metal. Another employee was working on the ground as a gopher. Employee #1 used a 40 feet, Louisville, aluminum, extension ladder, Model: L-2121-40, to access the roof. Employee #1 had sealed the small opening between the roof trim and the flashing around the left and center dormers on the upper roof. Employee #1 moved over the right dormer and approximately five feet from the upper roof edge. Employee #1 informed the other employee of misty rain falling. Employee #1 slid off the upper roof and landed on the lower roof unknown distance below. Employee #1 continued to slide approximately 12 more feet towards the lower roof edge. Employee #1 landed head first onto the ground. Employee #1 sustained blunt trauma to head and neck with skull fracture, swelling, and deformity to the head and scalp and was killed.
