Investigation Summary
At 9:00 a.m. on August 22, 2018, Employee #1 was using an Ingersoll Rand commercial air compressor that was attached to the work truck and an airline with a metal 8 inch long nozzle to feed a woven tracer tape into underground conduit approximately 600 feet up the road to be connected to a fiber optic cable. Employee #2 was holding the tracer tape so that it would not tangle up as it was being fed into the conduit. Both employees were electrocuted when the nozzle kicked back and the taper tape struck an overhead powerline. Employee #1 was killed; Employee #2 suffered third degree burns and had his right hand amputated due to the severity of his burn.
