Investigation Summary
At 2:10 p.m. on December 12, 2017, an employee was working inside a trench that was approximately 20 feet deep installing a 30-inch by 15 foot long PVC sewer pipe. The employee was standing inside an 8 foot tall by 24 foot long trench box. A coworker placed a steel sling in the middle of the pipe as the excavator operator lifted the pipe to level it before moving towards the trench. The coworker let go of the pipe after it was leveled and the wind caught it, pushing it towards the trench. The pipe slipped out from the sling and fell downward into the trench box where the employee was unable to escape from its path. The employee was struck on the back of his head, neck and back, killing him onsite.
