Investigation Summary
At 11:55 a.m. on June 11, 2018, a 51 year old construction laborer was stocking a scaffold platform with concrete blocks. The employee jumped from one scaffold platform to a lower platform. When the employee jumped to the lower platform he lost balance and fell into an elevator shaft. The employee fell approximately 25 feet. The employee sustained multiple fractures and died.
