Investigation Summary

At 1:45 a.m. on January 4, 2019, an employee was loading packages inside a trailer and the extendible conveyor engaged and moved towards the employee. The employee's back was turned towards the conveyor when the conveyor pinned the employee against the packages. The employee's coworkers were alerted by the maintenance monitoring system and "light" monitoring system at which time they found the employee. The coworkers were unable to access the conveyor's controls because they were pinned between the employee's lower back and the boxes, so they proceeded to 'dig out' the victim by removing the packages. Once freed, the employee's coworkers provided first aid until emergency medical personnel arrived and transported the victim to the hospital. The employee was pronounced dead as a result of the injuries sustained in the fatal incident. Just prior to the incident, security video showed the employee controlled extendable conveyor slowly extending inwards into the trailer where the employee was located.
