Investigation Summary
At 10:00 a.m. on July 9, 2018, Employee #1 was unloading steel from the forklift. The employee reached over to the right side of the cab for some paperwork, and actuated the boom lowering lever. He was struck by the boom, crushing his neck and upper torso, and killing him.
