Investigation Summary
At 3:00 a.m. on April 25, 2018, Employee #1 was traversing through the receiving loading dock area of a facility when a jockey truck (powered industrial truck) collided with him. The front driver's side of the vehicle ran over Employee #1. Employee #1 suffered multiple body trauma and was killed.
