Investigation Summary
At 3:15 p.m. on June 14, 2018, Employee #1 was driving a forklift, unloading utility trailers that had been delivered by a semi-trailer truck. The utility trailers came loose from the ball that was attached to the forklift and started to roll into the street. Employee #1 exited the forklift cab to attempt to stop the trailer from rolling into the street and was struck and killed by the forklift. He sustained a fractured pelvis and multiple internal injuries.
