Investigation Summary
At 6:30 p.m. on July 13, 2017, an employee was directing traffic at the intersection of FM 2208 and FM 450 controlling equipment movement inside the work zone and ctivity area. While multiple roadway equipment were moving in a linear convoy formation from the East of FM 2208, heading North on FM 450, the employee attempted to cross between the moving Asphalt Distributor Truck and the Chip Spreader. The employee was run over by the Chip Spreader. Employee sustained injuries to the head and internal organs and was killed.
