Investigation Summary
At 9:45 a.m. on January 16, 2018, an employee was loading fertilizer and was operating a skid steer when the bucket of the skid steer froze in the raised position. The employee went to pick up a hammer and walked back under the skid steer bucket when it fell and struck him. The employee was killed. A second employee was injured during the incident and was hospitalized.
