Investigation Summary
At 5:00 p.m. on June 14, 2017, Employee #1 and two coworkers were serving guides on a kayak trip of the Yellowstone Lake. While paddling in open water, one of the clients had fallen into the water. After Employee #1 and the two coworkers rescued the client, they began pumping water from the clients overturned kayak. When this was not successful, Employee #1 and the coworkers decided that the client and their kayak needed to be towed to shore. One coworker tied the clients kayak to theirs using a through bag and began paddling to shore, while Employee #1 and the other coworker followed behind in their kayaks. During transit to shore, Employee #1s kayak rolled over and dumped Employee #1 into the water, who suffered hypothermia and was killed. The incident investigation revealed that Employee #1 had a hyperemia kit containing a wool blanket and cell phone to call for emergency services and was wearing a personal flotation device at the time of the incident.
