Investigation Summary
At 10:30 a.m. on August 29, 2019, Employee #1 and a coworker, employed by a construction company, were handling structural steel beams for a two-story building, a multiemployer construction project. Employee #1, working from a flatbed truck, was handing I-beams to his coworker on the street. Employee #1 fell. As he tried to hold the unsecured I-beam, the beam fell and struck his forehead. Employee #1 was killed.
