Investigation Summary
At 10:30 a.m. on March 8, 2018, Employee #1 was shimming a 2 x 4 underneath a rack stand while the subordinate coworker was hitting the stand with a sledge hammer. The rebar cage shifted causing the rack stand to fall over, trapping Employee #1 between the rebar cage and the horizontal beam of the rack stand. Employee #1 was crushed in the torso area and was killed.
