Investigation Summary
At 8:18 a.m. on January 8, 2018, an employee was working alone in the County Athletic Center maintenance area. The employee utilized an 8 foot stepladder to attach a wire and thermostat to the conduit of a ceiling mounted Reznor heater 12 foot 9 inches above the next walking working surface. The employee stood on the top step and fell 6 foot 8 inches, and landed on the ladder with his back to the ladder fatality and was killed.
