Investigation Summary
At 7:15 a.m. on February 22, 2019, an employee was located on the roof of a two-story duplex apartment to survey the condition of work that had been completed. The building overlooked a courtyard, which the employee set up an extension ladder to access the roof. Before descending the ladder, the employee was speaking with his son who was working on the roof of another building that shared the same courtyard. The employee lost consciousness and fell forward off the roof to the courtyard and was killed.
