Investigation Summary
At approximately 9:15 p.m. on January 29, 2019, an employee was walking on the snow and ice covered island, slipped and fell, hitting his head. The employee was on the ground for approximately 10-12 mins, and when he got up, he appeared lethargic. The employee paced around two semi-tractors for approximately 2 hours, before collapsing to the ground. The employee was found unresponsive between semi-tractors. The Medical Coroner's Report states immediate cause was blunt trauma of the head, due to or as a consequence of a fall.
