Investigation Summary
At 8:15 a.m. on March 2, 2020, an employee was coupling a removable gooseneck low-bed trailer. The employee reattached the gooseneck to the trailer and then connected the air and electrical lines. Shortly thereafter, the tractor and trailer rolled backward and stopped after striking a parked vehicle that subsequently struck a parked trailer. The employee was fatally injured after being crushed under the tractor's driver's side rear dual tires. The tractor's parking brake system had not been engaged; therefore, when the employee connected the air lines to the trailer, the trailer's air brake system did not engage. Additionally, the tractor and trailer were on an incline (approximately 3-5 degrees), and wheel chocks were not used to prevent movement. The tractor-trailer may have rolled back when the employee operated a lever to raise the trailer deck off the ground, which would have been the next step in the process after connecting the air and electrical lines, and the employee typically stood between the tractor's rear dual tires and the trailer deck to operate the lever. There were no witnesses to the incident. The employee was killed as a result of multiple blunt force injuries and mechanical asphyxia.
