Employee Is Struck And Killed By Excavator Bucket

At 10:30 a.m. on January 19, 2022, an employee disconnected a chain hook from a concrete storm drainage pipe that was lowered into a dump truck bed by a coworke r operating an excavator to move the pipe. The employee was in the rear inside p ortion of the bed as the coworker leaned out of the cab to attempt to get a bett er view of the employee. The operator had the engine at idle and contacted a con trol lever that caused the excavator bucket to move and pin the employee between the dump truck bed and the bucket. The employee was crushed and was killed by t he internal injuries received.
