Investigation Summary
At 5:19 p.m. on October 5, 2020, an employee, a quality control seasonal employee, was walking back to her work station after her lunch break. The employee's path took her from the break area around the front of a truck unloading potatoes. As the employee stepped in front of the truck, it pulled forward intending to turn to the left to drive away. The truck driver never saw the employee as she was not visible over the front of the truck's hood. Employees were not required to wear high visibility clothing and there were no barriers, designated pathways or other controls to keep employees from walking directly in front of trucks. The employee was struck by and crushed under the right front tire of the truck. The employee suffered multiple crushing injuries, resulting in death.
