Investigation Summary
At 3:00 p.m. on October 11, 2018, an employee was loading steel beams onto a goose neck trailer and was using a pry bar to straighten out an I- beam. The employee lost his footing, fell to the ground, and was struck in the head by one of the steel beams that fell from the trailer. The employee was killed.
