Investigation Summary
At 4:00 p.m. on October 6, 2017, two employees were attempting to remove sludge from a wood chipper's fuel tank. An explosion and fire occurred inside a metal building. The weather was 81 degrees, 10 mph south wind, clear skies. There were no witnesses. The Fire Marshal observations was that one employee was found with tools and the remaining parts of a torch near him. The building was totally destroyed. An ignition source lit vapors causing an explosion and fire. Both employees died from asphyxiation.
