Investigation Summary
On August 18, 2017, Employee #1 was pumping hot oil from fryer 1 to the oil storage container. The oil storage container was located in the next room opposite the batter and baking area. While pumping the oil from the fryer, Employee #1 was setting up for the second part of the process. Employee #1 was connecting a pipe to the front of the fryer that would allow sediment to be washed from the fryer and into the drain with water once all of the oil was pumped out. Employee #1 walked into the next room where the oil was being pump into the oil storage container. Employee #1 went to retrieve a clamp from a tool cart to connect the pipe. While at the tool cart, Employee #1 noticed something was wrong with the pumping of the oil and ran to the oil storage area to turn off the valve that allowed the oil to be pumped into the oil storage container. While turning the valve off, the oil storage container erupted and overflowed onto Employee #1. Employee #1 was completely engulfed in hot oil. The oil temperature at the time when it overflowed was between 350 and 400 degrees Fahrenheit. Employee #1 was transported to a burn center. Employee #1 expired two months later on November 4, 2017 as a result of the burns sustained to his body.
