Investigation Summary
At 3:15 a.m. on November 14, 2018, an employee was spotting for the steel side sheet and fell onto the roller bed. The employee's head and neck hit the roller and it appears that the roller grabbed her jacket. She sustained internal head trauma and later died at a local hospital.
