Investigation Summary
At 9:30 p.m. on March 16, 2019, an employee had filled his tractor trailer with Lime/Gypsum and had shutoff the feed. The employee returned to Pugger floor where he fell through a floor opening where an unsecured section of grating had been in place. The floor grating slipped from cross members it was setting on, creating a 30 inch by 50 inch floor opening. The employee fell through the opening and was killed.
