Investigation Summary
At 3:45 p.m. on February 22, 2019, Employee #1 and a coworker were working on the sloped roof of a single family home. They were applying a Tyvek vapor barrier around a newly constructed addition to the existing home by installing a window to the second level. The worker was holding one end of the Tyvek paper, while Employee #1 rolled the Tyvek paper down the roof to cover the addition. During work, Employee #1 lost his footing on one of the walk boards and fell backwards from the roof. The employee fell a distance of 12.58 feet and struck a concrete patio, killing him.
