Employee falls from scaffold and is killed

At 4:00 p.m. on March 1, 2018, Employee #1 was building a scaffold platform that was approximately 52 feet high. During work, Employee #1 fell to the ground and was taken to a nearby hospital where he was declared dead on arrival. The incident investigation determined that Employee #1 was wearing a PFAS; however, there were no eyewitness to the incident.
