Investigation Summary
At 5:00 a.m. on March 6, 2019, Employee #1, employed by a refrigeration and heating company, was working at a multiemployer construction project, a retail store. He was climbing up and down a ladder as he disassembled electrical components in old walk-in freezers and assembled the electrical components for new freezer units. Subsequent examination of video cameras showed that a flash of light occurred and Employee #1 fell, along with the ladder, a fall height of approximately 10 feet. Employee #1 was killed. It was determined that he had sustained a skull fracture, brain injury, and electrical shock. Employee #1 had been exposed to live electrical parts. He had not been trained on the use of ladders, electrical safe work practices, and locking out/tagging out equipment before handling.
