Investigation Summary
At 12:30 a.m. on September 12, 2018, an employee walked over to talk to coworker #1 to see why construction vehicles were to be moved. After the conversation, the employee left the area to return to his post. Coworker#2 was in the water blaster vehicle, backing up to move out of the way so that coworker #1 could move his construction vehicle. Coworker #2 had to remove lane lines with the water blaster vehicle which were located under coworker #1's vehicle. While the construction vehicles were relocated, the employee was struck and run over by Coworker #2's vehicle. As a result, the employee was killed.
