Investigation Summary
At 8:45 a.m. on September 25, 2020, Employee #1, the company owner, was attempti ng to replace a seal on a hydraulic component that lifted and lowered a shipping container trailer bed. Employee #1 removed a pin that made the rear axle of the trailer bed stationary when the bed was loaded or unloaded. He then asked the t ruck driver to operate the controls so he could pull out the hydraulic component . When the controls were operated, the rear axle and tires attached sprung forwa rd rolling over Employee #1. He was crushed by the weight of the trailer bed and the 10,000-pound shipping container that was on the bed at the time of the acci dent.
