Investigation Summary
At 2:00 p.m. on March 10, 2018, an employee was pulling the exhaust manifold from a 2003 Tundra truck that was being elevated by a scissor jack. During work, a malfunction occurred, causing the jack to tilt toward the back of the truck. The employee became trapped beneath the truck and suffocated from the weight if the vehicle.
